Patients Practice Charter

We would like to introduce our practice charter to you. It is a statement of your rights and responsibilities as a patient of this practice. We are keen to work with you in providing the best possible care available.

We aim to provide the following care:

  • To offer a health check on registering with the practice as a new patient
  • To offer a copy of the practice booklet, setting out the services your doctor provides
  • To see urgent cases at the next available surgery
  • To refer you to a consultant acceptable to you when the doctor thinks it is necessary, and to be referred for a second opinion if you and your doctor agree this is desirable
  • To facilitate access to your health records, subject to any limitations in law
  • To ensure that those working for the practice keep the contents of your health records confidential - they are under a legal duty to do so
  • To provide a full and prompt reply to any complaints you make
  • To provide an explanation if appointments are running more than 30 minutes late
  • To allow you to choose whether or not you wish to take part in medical research or medical student training

Your responsibilities are:

  • To arrive on time for your appointment and to make a separate appointment for each person attending
  • To let us know as soon as possible if you are unable to keep an appointment
  • To ask for home visits only when absolutely necessary, and before 10.00am when possible
  • To give 2 working days notice for repeat prescriptions
  • To tell us immediately when you change name, address or telephone number
  • To understand that the results of tests are only given to the person who had the tests or in the case of children, the parent or guardian
  • To ask about anything you do not understand
  • To treat staff politely and with courtesy as you would wish to be treated yourself
